PSYCHOSOCIAL REHABILITATION COUNSELOR

Assist in the provision of rehabilitation services, including educational activities, rehabilitation, and care services designed to develop, maintain, or restore the skills necessary to allow the consumer to function successfully in the community. A Bachelor's degree in the field of counseling, social work, psychology or a related human services field. Demonstrated ability to communicate effectively in oral and written forms. Being certified as a Behavioral Health Technician or Behavioral Analyst preferred. Three years of experience preferred. Certified Addictions Professional a plus. Facilitates groups designed to meet social, educational, skill building and therapeutic needs of the clients utilizing evidence based practices. Must have experience with the Adult SPMI population. Successful completion of Background, MVR, References and Drug Free Workplace screening required. Must have a valid Florida driver's license.

BEHAVIORAL HEALTH COUNSELOR

Master’s or Bachelor’s degree in the field of counseling, social work, psychology or a related human services field.  Florida license eligible registered intern, certified mental health professional, and/or certified addictions professional preferred.  Provides screenings, assessments, diagnosing, group and individual counseling, treatment planning, implementation of treatment plan, educational services, crisis intervention, psychopharmacology, collaboration with other agencies/service providers and documentation of all services.  Demonstrated ability to communicate effectively in oral and written forms.  Full-time position. Successful completion of Background, MVR, and DFWP Screenings.

SOAR CASE MANAGER

Must have a minimum of a bachelor’s degree from an accredited university and at least two years full time experience in working with adults who have serious mental illness and children who have serious emotional disturbances, also experiencing homelessness. Also, looking for certification through Florida Certification Board as Certified Case Manager. Experience in providing and advocacy services to clients in an outpatient mental health setting. Excellent written and verbal skills necessary. Experience as SOAR processor and assisting individuals experiencing or at-risk for homelessness who have mental illness and/or co-occurring substance use disorder or other medical impairments apply for Social Security disability benefits. Responsible for ensuring continuity of care through advocating, planning, linking, coordinating, and monitoring of services and resources to help improve the quality of life for clients served. Certification or certication-eligible by Medicaid as a Case Manager. Full time position. Successful completion of Background, MVR, and DFWP Screenings.

TARGETED CASE MANAGER

Responsible for ensuring continuity of care through advocating, planning, linking, coordinating, and monitoring of services and resources to help improve the quality of life for clients served. Must have Targeted Case Management certificate. Must be proficient in electronic health records system. Have a minimum of a bachelor's degree from an accredited university and at least three (3) years full time experience working with children who have serious emotional disturbances or two (2) years full time experience in working with adults who have serious mental illness or a Bachelor's degree from an accredited four year college or university with emphasis in the areas of counseling, psychology, social work, health education or related human services field. Minimum of one year of full time or equivalent experience working with adults and/or children who have serious mental illness and children who have serious emotional disturbances. Successful completion of Background, MVR, References and Drug Free Workplace screening required. Must have a valid Florida driver's license and excellent driving record.
