Case Management services are designed to assist clients (adults and children) in linking and accessing needed medical, social, educational, and other services beyond the mental health services available through Sutton Place Behavioral Health. Case Management services consist of services and activities provided to clients with serious and persistent mental and emotional illnesses, in order to help promote their highest level of functioning in the least restrictive environment.

Eligibility/Admission Criteria

• Adults and or children residing in Nassau County with persistent mental and emotional disorders are eligible for Case Management Services can be self-referred, referred by professionals in the medical, psychiatric, education and human services fields, and the court and legal systems.

Goal:

• The goal of Case Management is to aid clients in achieving the highest-level possible of success and satisfaction in the social environment of their choice with the least amount of professional assistance required. For children, this focuses on assisting the family unit to remain successfully intact with the least amount of professional assistance required.
