BEHAVIORAL HEALTH THERAPIST
Master’s degree in the field of counseling, social work, psychology or related field. Demonstrated ability to communicate effectively in oral and written form. Addictions certification required or in process of obtaining certification. Completes screenings, assessments, diagnosing, group and individual counseling, treatment planning, implementation of treatment plan, educational services, assessing effectiveness of services, crisis intervention, psychopharmacology, collaboration with other agencies/service providers and documentation of all service.
CASE MANAGER
Bachelor's degree from an accredited four year college or university with emphasis in the areas of psychology, social work, health education or related human services field. Minimum of one year of full time or equivalent experience working with adults who have serious mental illness and children who have serious emotional disturbances. Must have experience in providing case management and advocacy services to clients in an outpatient mental health setting. Responsible for ensuring continuity of care through advocating, planning, linking, coordinating, and monitoring of services and resources to help improve the quality of life for clients served.
