ACCESSING SERVICES

How to Access Services at Sutton Place Behavioral Health, Inc. 

To access services during regular office hours (M-F; 8-5): 

  • Come to the Agency to request an appointment.
  • Phone the Call Desk at 225-8280 x 448 during regular business hours to request  an appointment; calls for routine services received after regular business hours  will be returned the next business day.
  • Complete the Online Intake forms prior to your scheduled appointment.  Meet with an Intake Specialist for your financial intake and then meet with your clinician for your initial appointment.
  • An assessment with a counselor will be done and you will work with your counselor to develop your treatment plan.

To access crisis services during regular office hours (M – F; 8-5): 

  • Come to the Agency or call  225-8280 x448 during regular business hours to request a crisis appointment.  Clinically trained staff will assist in getting the services needed as quickly as  possible.

To access crisis services after Hours (after 5pm; on weekends): 

  • Call the agency telephone number 904-225-8280.
  • Ask to speak to the agency’s on-call counselor.
  • The on-call counselor will call you back to assist in the crisis.

Call 911 when:

There is an emergency that threatens your life or health or that of others.
