Socialization Programs


Psychosocial Rehabilitation and Drop in Center programs are designed to help persons with severe and persistent mental illnesses improve their quality of life in the areas of self-care, interpersonal relationships, health, leisure and vocational skills. The program provides intervention and support toward preventing escalation of symptoms, frequent hospitalizations, and/or the need for more restrictive residential placement.

Eligibility/Admission Criteria

  • To be eligible for services a person must be 18 years of age or older, and have a documented diagnosis of a chronic, persistent mental illness, and be stabilized on medications upon entrance into the programs. Participants are referred to socialization programs by mental health practitioners, case managers, or physicians.

Goals

  • The Goal of Psychosocial Rehabilitation is to empower individuals to make healthy choices on their own behalf and to develop independent living skills.

Services

Our socialization program offers the following groups:

  • Daily Living Skills: addresses food, nutrition, shopping, housekeeping, money management, safety skills and other skills necessary for eventual independent living.
  • Interpersonal Skills: Cooperation, communication, intimate relationships, problem-solving, and decision making.
  • Health and Wellness: Addresses symptom/illness and stress management, exercise and fitness, first aid, and relaxation.
  • Vocational: Topics include job readiness, task skills, letter writing, improving concentration, resume writing, internet job searching, and community resources.
  • Leisure: Opportunity to participate in activities such as sports walks, gardening, as well as hobbies and crafts of their own choosing. Structured social outings to various points of local interest.
  • Supported housing and supported employment services.
