You can give our office a call at (904) 225-8280 and follow the prompts to establish care. Once you are connected with a member of our Registration Team, you can sign-up for our patient portal and complete all pre-registration forms. You can also complete any registration when you come in for your first visit.

Nope! You don’t need an appointment. We want it to be easy for you to get help. That’s why we have special times called “Open Access Hours” at three different places. Just come during those hours—we’ll be ready to help. Plan to be here for about 2.5 to 3 hours so we have enough time to get to know you and support your needs.

You can visit any of our three offices—pick the one that’s best for you:

· Callahan: 617225 Brandies Ave, Callahan, FL 32011

· Fernandina Beach: 1541 S 14th St, Fernandina Beach, FL 32034

· Yulee: 463142 SR 200, Yulee, FL 32097

You can either arrive at your reserved time, or each office has its own walk-in hours:

· Callahan: Wednesdays, 10 AM – 4 PM and Fridays, 8 AM – 12 PM

· Fernandina Beach: Fridays, 8 AM – 12 PM

· Yulee: Mondays, Tuesdays, and Thursdays, 8 AM – 2 PM

· A photo ID

· Any papers that show you’re the legal guardian (if you're bringing a child)

· Insurance information (if you have it)

Here’s what you can expect:

1. If you didn’t pre-register, we’ll ask you to fill out some forms.

2. You’ll answer a few questions about how you’ve been feeling—like if you’ve been sad, worried, or stressed. There are no right or wrong answers.

3. You’ll talk with a team member for about 15 minutes so we can learn more about how to help you.

4. We’ll also talk to you about costs, like insurance or co-pays.

5. Then, you’ll meet with a trained intake specialist for about 90 minutes to talk more about your goals.

6. We’ll give you a folder with helpful info and your next appointment date.
